Case 001 — Painful “pimple” in beard area

HPI: 24M, recurrent tender nodules in beard after shaving.

Exam: Follicular-based papules/pustules, some keloidal papules on nape.

DDx: Pseudofolliculitis barbae, bacterial folliculitis, acne keloidalis, hidradenitis suppurativa (rare in beard).

Plan: Stop close shaving, use clippers; warm compresses; topical clinda + BPO; if persistent, topical retinoid; consider oral doxy x 4–6 wks; address keloidal papules with ILK/laser if needed.
